Community Health Workers (CHWs) serve as trusted connectors between individuals, healthcare providers, social-service systems, workforce programs, and community resources. CHWs can help individuals: Identify social determinants of health and other barriers Develop individualized goals Navigate complicated service systems Connect with healthcare and community providers Access housing, food, transportation, and other resources Strengthen self-advocacy skills Understand available programs and benefits Coordinate referrals and follow-up Build skills for greater independence Stay engaged with their established care and employment plans CHWs are especially valuable when an individual is receiving services from multiple organizations because they can help bridge communication gaps and support continuity of care. The goal: Help individuals navigate systems, reduce barriers, and remain connected to the resources necessary for success.
